PHYSICAL THERAPY SERVICES ? Digos City in Digos City, Davao Region

Performed a wide range of duties with increased focus on in-home support and assistance for the ailing or elderly along with the development of senior's assisted living from personal care and home
management to health care procedures and assistance with movement and activities: Assist patients with walking, moving, proper positioning, maintaining body alignment or posture, do physical lifts and transfers, give range of motion exercises and other rehabilitation procedures, assist patients with personal hygiene, bathing ,dressing and feeding, take and report temperature, pulse, respiration, body weight, collect urine, sputum and stool specimens, assist with colostomies,ileostomies and ureterostomies, observe and report all changes in behaviour or ongoing condition and other information pertinent to a patient's care.
DUTIES:
1. Direct/apply physical therapy services according to a written physician?s plan of care. These services may include but are not limited to:
A. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
B. Assesses for muscle strength, mobility, gait, ROM and rehabilitation potential.
C. Directs/apply physical therapy treatment.
D. Instructs patients and families/caregivers in the use and care of therapeutic appliances.
2. Delivers services and treatments to patients in accordance with accepted standards of treatment specifically effective for the condition(s) which the patient suffers.
3. Participates in client case conferences and in service programs to ensure coordination of patient care.
4. Assists with the development and implementation of the plan of care in collaboration with the physician and other members of the health care team
5. Initiates physical therapy program and instructs other personnel and/or family members/caregivers in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient.
6. Maintains appropriate and comprehensive clinical records documenting treatment rendered, patient limitations and progress and any communications with other members of the health care team.
7. Prepares and submits clinical and progress summaries in a timely fashion in accordance with policies and procedures.
8. Trains the patient in the use of a prosthetic device as indicated.
9. Assists in the identification of patient and family/caregiver needs for other home health services and refers for such services as indicated.
